Forms We Need
Before Dr. Baller treats you, we need some information to establish you as a new patient with us. We require your dental and medical history, and we also need a complete listing of all your prescription medications.
Please visit our forms page to download, print, and complete the information you need beforehand and bring it with you.
Remember to bring your insurance card or any credit cards you plan to use for payment.
Your First Experience at Baller Dental
Your first experience at Baller Dental will typically start with one of our skilled dental hygienists unless there’s a specific concern you’d like to share with Dr. Baller.
Our hygienist spends the first few minutes chatting with you about any oral health concerns you have. If you don’t have current x-rays, we will take new ones to have a complete and updated picture of your oral health.
We assess your current oral health needs through a thorough examination and then perform a teeth cleaning; the exception is if we find gum disease that requires a separate, longer appointment to treat.
If we find issues that we need to address, we’ll work with you to develop a step-by-step treatment plan to restore your smile to optimal oral health.
Before You Leave Our Care
Before you leave our Aurora dental office, a helpful team member will explain your insurance benefits and describe our dental savings plan and the financing options we offer. We will also help you schedule appointments as needed.
